Please Read: To schedule an appointment, please call our office at 515-222-1911 or use the following instructions and link to schedule online.

Log into your online scheduling account (if you have not created one, set it up by clicking the first-time user option).
Select the person you are scheduling for (self or family member).
Select the visit type you are seeking and the doctor you prefer. We recommend continuing with the same doctor unless there is a major scheduling conflict.
Pick the available time that works with your schedule and confirm your appointment
You will receive an email that indicates if it has been accepted or denied. Please review this to make sure the appointment has been successfully scheduled!
